Transitional Services
Transitional Services programs support youth and their families during transition in their lives to ensure productive, positives futures. Youth are referred to the PATHS Transitional Services program by the Division of Youth Corrections and referred to the Out-of-Home Placements program by County Departments of Human Services.
PROGRAMS
• The PATHS Transitional Services program matches a Case Manager with a paroled youth to support his or her transition back into the community. The goal of the program is to ensure the client’s successful re-entry into the community.The program serves approximately 80 youth between the ages of 13 and 20 annually.
• DAYS Out-of-Home Placement programs serve children from 0-18 years of age, with a primary focus on pre-adolescent and adolescent youth. The goal of each program is to provide youth with a safe and supervised home that is structured with age appropriate boundaries and expectations. While in placement, each youtng person is provided the opportunity to learn and practice the skills necessary to better manage their behavior and create personal success in their lives. Each year, the Foster Care program serves 45 youth, the Proctor Care program serves 25 youth, and the Kearney Street program serves 25 adolescent mothers.
