The Case Management & Mentoring Relationship Roles:
The PATHS Transitional Services Program for
DYC Parolees offers more
than a menu of services to our young clients. We provide the quality
individuals who are behind the services. Upon parole, the PATHS Case
Manager is probably the most available and is possibly the ‘key’ person
a client can lean on and learn from in the brief period prior to
expiration of his or her commitment. Each has their own particular
assets and all are highly skilled. We have Spanish-speaking, CAC
certified and sex offender trained staff.

PATHS Parole Transition Program Staff
are exceptional in many ways – they have years of experience and
background in working with at-risk young people and are dedicated as
well as invested in promoting client success. Our Case Managers take
pride in seeing their efforts result in the clients’ forward progress
and they go ‘above and beyond,’ attempting to move their clients toward
success every time. Our case managers assume the position of mentor to
the client in order to establish the youth’s experience of a healthy
adult relationship. The same case manager sees that the client follows
terms of his or her parole plan, insisting on compliance and following
up with prescribed consequences - a delicate balance, quite a skill and
an enormous learning experience for any youth needing support and
guidance (& often providing valuable role modeling for parents,
themselves).
Expectations of PATHS Case Managers >>
Providing self as model for a healthy lifestyle; assisting youth in
identification of goals and in goal attainment; motivating clients
toward change; offering opportunities for education, productivity
& work; validation of improvements & celebration of
successes; assisting youth with overcoming difficulties &
hardships; showing clients how to tackle difficult barriers &
problems; providing methods of appropriate crisis management; teaching
approaches to establishing & nurturing healthy relationships,
boundaries & the necessity of letting go when appropriate;
assisting youth to understand the value of significant positive
& loving relationships; guidance toward positive well-being;
assisting with spiritual pursuits, clean fun, & recreational
experiences.
Processes for accessing services >>
Initially,
the PATHS Transitional Services Program Manager, following receipt of
an approved referral form, will review the referral and accompanying
documents and will assign the client to an appropriate PATHS Case
Manager.
Progression of services is as follows: >>
The Case Manager will review the referral packet for requested services
and other relevant information and will make contact with the DYC Client
Manager /Parole Officer, within 48 hours of assignment.. The Case Mgr.
will gather information pertaining to upcoming appointments, meetings
and case plans. Once services have started, the Case Mgr. will contact
the DYC CM/PO weekly or, if needed, more frequently. Discussion will be
directed toward client progress or regression, problems or issues and,
as necessary, to fine-tune and/or adjust case plans or services.
PATHS services have historically begun at the time of the youth’s
parole hearing, however, the Case Mgr. is prepared to begin an earlier
involvement with the client in order to establish a positive
relationship with the youth prior to release onto parole status. The
PATHS Case Manager will be present at the youth’s Parole Hearing, as
specified by the DYC CM/PO.
As in the past, the PATHS program will provide Specified Individual
Services as identified by the Client Mgr. /P.O. PATHS will also provide
Service Packages. These packages of service options and levels will
allow the Client Mgr./P.O., the PATHS Case Manager and the client
him/herself to identify and focus on specific areas of need. In
addition, these packages allow for flexibility in service options as
the youth’s needs and abilities fluctuate and change.
